What Causes Severe Depression Followed By Sense Of Happiness?

I go through stages of self-loathing, hatred, depression every 3-4 months it seems. Immediately following these phases is a short period of extreme happiness and elation. Not euphoria exactly, but relief of the depression and stress that had been building up over the previous months. This doesn t last long before I return to a state of normality that lasts for another 2-3 months before the cycle repeats. Im on alprazolam 1mg PO 2x daily for anxiety and panic attacks (which happen more than often). Is it bipolar? What could be the issue?
