What Causes Severe Lower Abdominal Pain Near The Groin Area?

43 y/o female, G3, P2, A1, with recent onset of intense pinching like sensation in left lower abdomen, more toward the groin area. The pain isn t constant but is severe when it happens. History of right salpingo-oopherectomy in 1998 due to repeated cysts (largest and final cyst 16 cm x 9 cm x 9 cm). Ultrasound in 7/15 showed left ovarian cyst. I was told it would resolve on its own and was nothing to worry about. Periods have been coming more often and much heavier than normal for the past 2-3 months. Earlier today, the pinching pain became severe and continuous. CT scan on 7/15/16 (in ER for abdominal pain, nausea and vomiting) showed left ovary enlarged at 4.3 cm x 4.2 cm x 4.7 cm. No mention of the prior cyst although I would not consent to contrast administration. Dx with gastritis and discharged. I do not know whether to go to urgent care or wait it out. The pain is intense, not relieved by Ibuprofen 600 mg. I really cannot afford more treatment if not necessary but do not know if this could be a possible torsion. I keep telling myself that a torsion would be unbearable pain but must admit that I did not notice the previous large cyst on the right ovary before removal. I have had some nausea but no vomiting and no fever. Is the enlargement of the ovary significant? Should I be concerned about torsion?

Hi,

Noted your detailed history about having operated right salpingo-oophorectomy for a very large cyst, persistent pain in the left groin area not responding to Ibuprofen 600, CT scan showing large left ovary but no cyst. Discharged as gastritis, pain continues.
The possible causes I can think about area as follows:
Torsion of an enlarged ovary, recurrence can pain that is not controlled with Ibuprofen. This can be diagnosed only if one goes for ultrasound with color doppler during stage of pain.
If found so then fixation of ovary can help a lot by avoiding torsion.
This can be done by Laparoscopy.
Another reasons can be hernia in groin that is missed as you have pain in groin.
Colitis is missed by CT scan and can be diagnosed by clinical suspicion and colonoscopy and the pain is relieved by antispasmodic.
Consult a General surgeon for second opinion.
Consult your Gynecologist for menstruation related problems and see if can be cured by hormonal treatment.
