What Causes Severe Lower Back Pain And Tingling Sensation In Legs?

I have a lot of lower back pain and it's gotten to a point that I have pain in my buttocks and my thighs and numbing tingling pain shooting down my legs and toes. I have had physical therapy and I have had spinal injections which no longer help. I recently have the results of my MRI and I was curious as what it means and if surgery will help?

I have mild disc bulge and bilateral facet arthritis & mild spinal canal narrowing at L2 and L3

I have mild generalized and left posterior lateral disc bulge and bilateral facet arthritis with mild spinal canal narrowing at L3 and L4

At L4-L5 I have disk space narrowing in, Modic type one degenerative in plate change, generalized disc bulge, and moderate facet arthritis with mild to moderate spinal canal narrowing and severe right and mild left neural foraminal narrowing. There is encroachment on the right L4 nerve root in the neural foraman.

At L5-S1 there is sacralization of L5. The L5 – S1 disk space is dimentary.
