What Causes Severe Pain In The Lower Back?

My friend has lower back pain, she has complained now for over 6 months, she has been to several doctors, had MRI, cat scan and they have told her she has no slipped disk, no pinched nerve. Her husband just told me today she has two small knots now on her lower back, just off to one side of her spine, one he said is the size of a half dollar and protruding out about 1/2 . The second one is just a little lower than the first, not protruding, but sore to touch?
