Hi,

Low iron means anemia or exactly iron deficiency anemia. Severe stomach pain can be caused by a gastritis or a stomach ulcer. Ulcer can cause anemia and deplete the iron stores in the body. Metformin can cause gastrointestinal symptoms as side effects but in my opinion this is not your friend's case.

To resume my answer: In my opinion, your friend has a gastric ulcer that has caused iron deficiency anemia. A gatric fibrogastroscopy is necessary to confirm the diagnosis and than the doctor can start the treatment with a PPI drug and iron supplements.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
