What Causes Shortness Of Breath Along With Chest Pain And Tachycardia?

Hello, I m 23 years old,female. I feel shortness of breath and muscle pain in legs when I walk fast and climb stairs. This shortness of breath associated with chest pain and moderate tachycardia. The second floor I can go easily but from the third floor it starts. I couldn t find any diagnosis of this problem.can you please help me to find out the diagnosis and treat it.

Do you have any history for clotting disorders( blood too thick) , stroke, heart disease in your family history?
Do you smoke?
