What Causes Shortness Of Breath Along With Swelling In The Face?

My husband, 56 yr old smoker with poor eating habits, is experiencing shortness of breath. It started a day or two ago but is worse now and today I noticed that his face looks puffy around cheeks and eyes. He says his feet and ankles are a little swollen as well. He refuses to go to E.R. and has just now been able to go to sleep but I noticed him making a grunting noise before going to sleep. Should I call an ambulance? I see that it could one of NINETY TWO ailments but all seem to be very serious. I dont want to pay 15.00 to be told that he needs to go have tests done at the Dr. office. I know this but getting him to go is the problem.
