As you are a patient of hypertension, diabetes,
hypercholesterolemia,
hypertriglyceridemia as well as of asthma, now you are having a controlled BP, sugar, lipid profile as well as asthma, although, these illnesses might have caused structural changes in your body vessels including arteries of lungs, kidneys and heart.
Shortness of breath with early fatigue may be a sign of COPD, pulmonary hypertension, congestive
heart failure etc., due to different causes. Also, a raised creatinine level is pointing towards kidney involvement, which may be due to hypertension as well as diabetes. In my opinion, you need your evaluation of heart functioning by baseline ECG, TMT and
echocardiography as a first step to proceed further in your case.
