What Causes Shortness Of Breath, Lower Back Pain, Swollen Abdomen And Severe Fatigue?

I have been told that I have CHF; however one of my doctors is uncertain with this diagnosis. Today I tried working outside but had great difficulty due to shortness of breath, severe lower back pain, swollen abdomen, and severe fatigue. I pushed myself beyond my limits. When I came inside, I was so fatigued that I sat in my recliner and immediately fell asleep. When I woke, I had great difficulty breathing, and my abdomen was swollen even more. The thing that puzzles me is that my ankles and legs are not retaining all that much fluid. I am a diabetic, have been treated for high BP since age 28, and I am now age 70. I have also been diagnosed with mild pulmonary hypertension. I am presently using a CPAP machine. Thank you.
