What Causes Shortness Of Breath Upon Exertion?

70 yr old female with 1 week H/O SOB with exertion. NO chest pain, no edema, no fever. Has had a chronic tickle cough going on for awhile. EKG shows NSR with non specific ST changes in V leads (looked better than previous EKG). B/P was 134/94. Normal SAT and in no distress. On an ACE-I. Daughter keeps asking why she coughs all the time (before the SOB by the way). Changed ACE-I to an ARB (thinking may be ACE cough). Got Chest Xray-Shows LLL pneumonia with small left pleural effusion. Later got back labs-normal CBC, normal kidney function, lytes. But NT Pro-BNP shows 1584. Stumped??

We have to rule out possibilities like cardiac and pulmonary causes.
