What Causes Small Ulcer On Toe While Suffering From Cellulitis?

My 98 year old mother has been diagnosed with a stage of cellulitis on the inside of her big toe; ulcer about the size of the end of my little finger. Been healing pretty well; going on for a couple of months. Now there is a new small ulcer on the inside of the next toe. These are SO SO painful; she can hardly walk. She took 14 days of Keplex but it gave her uncontrollable diarrhea which results in a seizure (something she has never had before). So now they have prescribed topical Mupirocin which we have been using 3 days. Have not seen any improvement. What should we do?

I have gone through your query and being your physician i completely understand your health concerns.
For how long she suffering from it? Any other associated problem? like diabetes or hypertension? Any history of TB in past or exposure to TB? Are u running fever? Any other lymph node swelling in ur body?
Ulcer in foot need to be investigated for the cause. if it is secondary to trauma, or diabetes or due to some vascular pathology.
She need to be examined by your SURGEON so that she can be appropriately examined and investigated.
If i am your attending doctor i will get her FBC RBS ESR CRP and doppler ultrasound of the lower limbs . In light of these i will be able to offer you treatment.
Meanwhile stay calm and use acetaminophen to relieve pain. You should meet your surgeon so that if it need debridement ,it can be done timely along with proper antibiotics.
