What Causes Soreness Around The Penis Head?
The skin on around the head underneath my penis gets raw and very sore like once a month. I use like a antibiotic like cream when it occurs but I need to know why it continues to happen. It is not associated with sex. I am a diabetic and I am wondering if it is a side effect.

You have repeated balanitis inflammation or infection in once a month as per history
You are right... Diabetes patient tends to have more chance of infection as compare to normal population..
So control your diabetes with medication... You have not mentioned sugar value...
For balanitis if inflammation more than topical steroid cream might prescribed
If exudation present and bacterial infection suspected than antibiotic given
After sex wash genitals with soap and water
