What Causes Sporadic Testicular Pain?

Hello. This is a bit of a complicated one but here goes. I have had on and off groin and testicle pain for YEARS. Sometimes it goes away for weeks, sometimes months but it always comes back. Usually the pain is almost at the roof of my groin but my actual testicles sometimes get tender and mildly painful to touch. Never both at once but it happens to both, right mostly. Lots of walking and extended sitting can exacerbate it. I have a healthy son despite having the problem for years already so it doesn t seem to be affecting the baby gravy and I ve never seen blood in it or found lumps anywhere down there. There does seem to be a vein or something going across the right but not the left. It s just painful, unpleasant, and I m SICK of dropping my daks to self examine the damn things.

Hi,

You need to consult a surgeon and get tested. An ultrasound scan of the scrotum is required. According to your history it looks as if you may be having varicocele. Its an enlargement of the veins within the loose bag of skin that holds your testicles (scrotum). A varicocele often produces no signs or symptoms. Rarely, it might cause pain. The pain may:

i. Vary from sharp to dull discomfort
ii. Increase with standing or physical exertion, especially over long periods
iii. Worsen over the course of a day
iv. Be relieved when you lie on your back
v. Impaired fertility

Varicocele treatment might not be necessary. Many men with varicoceles are able to father a child without any treatment. However, if your varicocele causes pain, testicular atrophy or infertility or if you are considering assisted reproductive techniques, you might want to undergo varicocele repair.

The purpose of surgery is to seal off the affected vein to redirect the blood flow into normal veins. In cases of male infertility, treatment of a varicocele might improve or cure the infertility or improve the quality of sperm if techniques such as in vitro fertilization (IVF) are to be used.

Clear indications to repair a varicocele in adolescence include progressive testicular atrophy, pain or abnormal semen analysis results. Although treatment of a varicocele generally improves sperm characteristics, it's not clear if an untreated varicocele leads to progressive worsening of sperm quality over time.

Varicocele repair presents relatively few risks, which might include:

i. Buildup of fluid around the testicles (hydrocele)
ii. Recurrence of varicoceles
iii. Infection
iv. Damage to an artery
