What Causes Stomach Troubles And Fecal Stains In Uderwear?

My niece is 13 and always has large fecal stains in her underwear. Her father has Crohn's Disease and she has lots of stomach troubles that leave her in the bathroom for long periods of time. She gets angry when I even mention it, but it hasn't gotten better and it's been going on for years. Her dad is a single father and still just deals with it as a wiping issue, but she's a smart, sensitive girl and I know there must be more going on. Is there anything you can reccomend? They don't have insurance right now either.
