What Causes Swelling And Bleeding From A Scar In The Lower Lip?

I am living in a retirement residence . Am a former RN. One lady has a grossly swollen lower lip with a scar patch which bleeds. Some of the residents refuse to sit with her in dining room. She has been treating it on her own and refuses care . Could it possibly be caused by a virus? The condition has worsened over 12 months thank you.
