Hi docter... my friend have a teeth problem every changes weather, his gum will swallowed and pain. Today his gum bleeding and there's small hole in the different side. Would you tell me what his problem..? He is strong smoker , and dring coffee much.. Thank you very much. Regards Uli

As per your complain swelling and pain in gums along with hole over the gums and having a tendency of heavy smoking is pointing towards severe gun infection leading to inflammation of gums and causing swelling and bleeding..
I would suggest you to consult an Oral Physician and get evaluated and a clinical examination and a full mouth x ray can help in diagnosis and treatment can be done accordingly..
The first thing he has to do is stop smoking as it can further exaggerate gum problems.. Secondly he can be advised a deep scaling and Curettage followed by a course of antibiotics and painkillers for relief.. Massage the gums with gum paint.. Do warm saline gargle.. Antiseptic mouthwash gargle can be helpful.. Do cool compresses over the gums..
