What Causes Swelling In Hands And Lips Along With Upper Abdominal Pain?

My brother became ill last weekend, which began with swollen hands, lips and itching. They gave him benadyl and a steroid as blood tests came up negative at the ER. On Tuesday he thought he was on the mend, when yesterday he became very fatigued with severe upper abdominal pain. It was the same today and he went to the ER again and again, blood tests came negative. They gave him an antibiotic. They thought maybe the duodenum, but they seemed somewhat baffled. any ideas?
