Hi,

Since you are diabetic and HIV positive the immunity system is low and thus you can have any complications. Which in this case seems to be bacterial infection of the nose, sinuses and area around.
During Covid time loss of taste and smell is an indication that you can be Covid positive. Hence the first thing to do is to get RTPCR, related tests of blood and CT scan of nose and sinuses to know the underlying problem.
Continue Z pack till the full course is finished.
Consult your Doctor again for clinical evaluation and have added treatment in the form of another antibiotic, have proper doses of anti-inflammatory medicines.
Further treatment will be as per the findings and you should continue till recovered well.Antihistamine or decongestant along with multivitamins may be needed for symptomatic support as well as for proper healing.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
