What Causes Swelling In The Right Leg Along With Headache And Palpitations?

My husband is upstairs in bed while I am working in my home office. 42yo male, believe he has just hit his record high weight around 290lbs and is very frustrated by it but doesn t have the motivation nor energy to fix it right now. Family history of high cholesterol, high blood pressure, obesity and possibly diabetes. Began having cluster headaches in mid 30s, recognizes them and takes some sort of Benadryl cocktail as instructed by doctor. Has also been to the ER 2-3 times over 10 years when they were so bad he couldn t get rid of them. He takes 20mg Lipitor for high cholesterol, 300mg Wellbutrin extended release (150mg twice daily) for mood, 60mg Prozac at bedtime for mood. Had surgery Feb 20 to fuse together 2 wrist bones to one hand bone due to torn 1/4 ligament. Swelling is finally down, saw orthopedic today for cast, all is well. Has not been on Percocet nor any pain medicine for one week. Saw PCP late 2017 for extreme fatigue which had already been a problem for over a year but progressively gets worse. Constant drowsiness (works a desk job, sleeps on breaks, falls asleep on couch after arriving home from work, sleeps all night, cannot seem to sleep too much, constantly sleeping in a recliner and complaining how tired he is if I or one of our 5 daughters try to wake him). Dr suggested a sleep test first (due to my complaints of his snoring) then working on weight. Sleep test revealed sleep apnea. Put on a CPAP machine about a month ago. CPAP eliminates snoring, he tosses and turns less, but drowsiness isn t improving. Even seems to be increasing. Between the recliner and the bed, he spent most of the weekend asleep. He texted me a nearly an hour ago but my phone isn t making any sounds nor lighting up for texts/emails so I just saw his text. Here is what it reads...it is not typical of him to say just in case something would happen to me overnight : FYI-just in case something would happen to me overnight. My head is pounding. I took three ibuprofen. I feel acid coming up in my throat. I took Tums. I realized tonight the feeling I have in my right foot is from it being swollen. The feeling started yesterday but I didn t look at my foot until tonight. My chest feels like my heart is racing also. I m hoping that I just need rest. I love you. He does not suffer from reflux, and he had not said anything prior to tonight about his foot feeling odd. He did text me earlier this evening complaining that he has been on the CPAP for a month or so but still has no energy. He said he couldn t keep his eyes open and said I think it s time to contact my doctor. I was in a meeting but agreed he should contact his doctor asap. When I got home I saw him briefly, our oldest daughter s boyfriend showed up and asked for a photo session to celebrate an anniversary. I thought my husband went to the laundry room to work on laundry (which can take hours with our large family). I didn t bother him and started working with the kids. A few hours later, I realized he was asleep in our bed the whole time, not working on laundry. He had come downstairs and nonchalantly inquired about what would make one of his feet swell. I named a few simple ideas, he headed back upstairs to bed and sent me a text a little while later. He has not injured his foot, has not been lying on it funny, hasn t had one foot elevated more than the other, etc. (Just lying flat in bed.) Sat around all day yesterday (mostly sleeping) so both feet were elevated by the recliner or he was lying flat in bed. Any ideas? Should I be concerned? I m typically very confident in my assessments of our family s health and not a hypochondriac so any visits to the doctor are typically warranted and ER visits are rare because they must be something that definitely cannot wait until morning. With the texts he sent me, though, I m not sure if these symptoms warrant a visit to the ER. To summarize: **swollen right foot, began feeling odd yesterday but didn t notice swelling until today. **pounding headache (but did not say he has a cluster headache, which he would have mentioned) **feels like heart is racing (I don t own a stethoscope) **so drowsy he cannot stay awake (has been experiencing drowsiness for quite some time AND typically sleeps off a headache, but complained about drowsiness in a text 3-1/2 hours earlier without mentioning a headache) **acid reflux (we have been married 20 years and I ve never heard him mention acid coming up his throat)

Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

The presentation of narrated symptoms suggests that the drainage of particular limb is compromised due to various reasons giving swelling of isolated limb. Palpitation and headache are systemic features of stress, anxiety or others, which need a detailed case evaluation.

Thanks.

Take care.
