As per your complain a swelling under the tongue which is soft and fluctuant means that the swelling is a fluid filled and it can be a
RANULA which is a type of
mucocele and it is caused due to local trauma or injury to the
salivary gland ducts leading to spillage of mucous into the soft tissues leading to formation of a fluctuant swelling..
It can be due to a major salivary gland or a minor salivary gland..
In case of small swelling Marsupilization can be done while in case of large swelling surgical removal of
sublingual gland can be done..
The other part of the question is a swelling behind the ear can be a swollen lymph node and it can be due to
neck injury , or infections in the face and neck reason and at times infection in some other part of body can be also cause infection in distant lymph nodes..
At times a tumour of lymph nodes known as
Lymphoma which is a malignant condition can be a cause..
For
lymphadenopathy you should consult a physician..
