What Causes Swollen Feet And Ankle In The Elderly?

Yes, Thank you, I am a 69 year old female in excellent health, no heart issues(although family history of that), no high blood pressure, 145 pounds, 5.5 tall. Exercise 3x per week with trainer and always have extremely active throughout my life. Lately (6+ mo) I have almost everyday swollen ankles and feet, sometimes in the Am or during the day or at night. What could be the cause of this?

General & Family Physician 's  Response
Hello,
Thank you for your query.
I'm happy to note that you are taking good care of yourself and are active.

Edema (fluid retention) in the legs can be caused by multiple causes, the most significant of which are heart and kidney disease.

Other causes like venous insufficiency, Thyroid hormone imbalances, electrolyte imbalances, reduced protein or protein loss need to be ruled out as well.

I recommend :
-a physician consult
-blood tests (complete blood count, TSH, free T3, free T4, kidney & liver function tests, electrolytes, lipid profile, blood glucose levels) ;hormones (estrogen and progesterone levels)
-chest x-ray
-an electrocardiogram and echocardiogram to monitor heart function.
-an ultrasound abdomen to check for kidney size and other organ function.

Based on these results and clinical findings, we could arrive at a diagnosis and treatment plan. You may need more tests based on what we find.

In the meantime,
-avoid being on your feet too long at a stretch. Break every 15minutes.
-keep your feet above hip level when you rest to prevent pooling of blood. Make sure your whole leg is supported.(atleast below knee)
-avoid long hot baths.
-if you have noticed this swelling after a particular exercise routine, try to take it slow.
-if you have to sit for long, (like a flight or long drive), wear pressure stockings and do flexion-extension exercises to keep blood circulating in the flight or stop and take a stretch while driving.

If you notice any discoloration or experience pain, numbness or weakness, breathing difficulty, chest pain/discomfort, go to the ER right away.

Hope this helps. Pls let me know if there is anything else I can help you with.
Wish you good health
