What Causes Swollen Foot/forearm/arm/bicep/neck?

My wife has swelling in her right foot, fore arm, bicep, and neck line near her collar bone. No associative trauma that we are aware of. Episodes last 24-72 hours with a knot under the main area of swelling. Extreme pain and tender to touch is present.
