As per your complain swollen knuckles along with scrape on the fingers due to punching seems to be due to traumatic injury to the soft tissues leading to accumulation of inflammatory exudate as well as there can be
hematoma formation due to rupture of small blood vessels post injury.
Icing will help in reducing the inflammation and swelling but in case if there is any restriction in movement of the fingers or the movement is very painful there can be bony
fracture and you need to consult an Orthopedician and get him evaluated and an x ray can be diagnostic..
In case if there is bony fracture then reduction of the bony segments followed by immobilization using a cast or splint will help..
In case of only
soft tissue injury then you can continue with ice packs, give him
Ibuprofen to reduce pain and swelling..
Apply
Analgesic ointment over the swollen area..
