General Surgeon 's  Response
Hi,

The swelling below left ear that is not responding to antibiotic treatment can be due to the following reasons:
Tuberculosis as you have been advised Mantoux test.
Can be lymphoma is it is rubbery and not tender.
Parotid tunmor presents in a simlar way.
Hence to confirm:
Get the Ultrasound or CT scan of the area, this will give an idea whether this is a lymph node, its internal changes or a parotid tumor.
As per the findings FNAC that is fine needle aspiration cytology or biopsy can be done for confirmation.
Even an excision can be undertaken as it helps to get rid of the tumor and get a confirmed diagnosis.
Any additional treatment will depend upon the exact cause fiund.

Take care. Hope I have answered your question. Let me know if I can assist you further.
