What Causes Tenderness Around The Fingernails?

hi i m experiencing tenderness around my finger nails and they are also swollen. My palms have become dark and now my cheeks hv swollen and I have difficulty breathing due to a blood clot in my left lung. Any idea what could be the cause of this? Please help..
