What Causes Throbbing Head Pressure While Bending?

Have really throbbing head pressure (esspecially in front) when bend over. or lower my head.....within last year lost 75 pounds - current weight 191. No indication of cold or flu at this point, nose not runny...so i cant imagine it would be sinuses. Is it a potential heart attack sympton? Or brain head neck throat cancer/tumer? Male 51 year old smoker.
