General & Family Physician 's  Response
Hi and welcome to HCM. Teleact trio contains Hydrochlorothiazide 125mg Telmisartan 40mg, which is a good anti hypertensive. Your symptoms suggest that you could be suffering from some heart disease and also kidney problems.
Involuntary action in legs may indicate kidney disease. It is necessary that you have to undergo some tests like RFT, LFT and also it is better that you consult a cardiologist. He/she may suggest an angiography, which tells if you have any blocks in arteries in heart.
Blood pressure may varie in different arms and also in different times, Also cut down salt in your diet, restrict fatty foods, do some exercise.

All the best
