Hi.
Noted the history of a 59 years old male having
severe pain in abdomen, visited ER, said to have
bronchitis and given Z-pack.
The cause of severe upper abdomen pain can be one of the following:
Gastritis by viral infection or by Z-pack itself.
Referred pain from lower lungs or pleura can cause similar pains.
This can be diagnosed and treated by the following :
Get started with a PPI like
Pantoprazole and antacid, if the pain is subsided then this may be gastritis, continue these medicines and get an upper GI Endoscopy done to confirm if needed.
Continue treatment for bronchitis as before and get an X-ray of chest to rule out pleurisy or lower lung problems, many of these can be less symptomatic than the real severity as you are already taking medications.
Ultrasound of abdomen may be needed is anything is suspected for
gallbladder or liver problems, relevant blood and urine tests to be done.
Hope I have answered your query. Let me know if I can assist you further.

Dr. T. Chandrakant, General Surgeon
