What Causes Vaginal Spotting And Lower Backache?

Hi. I had my last period on apr 23 which was very heavy. Two weeks later i spotted for one day. Went to the doctor because i was experiencing lower back pain. I was prescribed a pain killer, which i only took for a day, and aminopep syrup. I also did a pregnancy test since i had been having unprotected sex 4 days straight up to my last period. The test was negative. Now i was expectng my period from may 19 but its not here. Whats wrong?
