Hello.

Possible causes of such a strong stomach pain with vomiting are severe gastritis, peptic ulcer or pancreatitis.

Of these peptic ulcer seems more likely as it can present as back pain and also can occur over a few days or weeks.

Pancreatitis is likely if she has any liver or gall bladder condition like gall stones or has an abnormally shaped pancreas or has an autoimmune disorder like lupus or rheumatoid arthritis. Pancreatitis can be confirmed by blood tests i.e. serum amylase and lipase and by ultrasound of the abdomen.

Your friend needs to be taken to the emergency as early as possible. all the vomiting may cause dehydration and electrolyte disturbances and aggravate her symptoms.

The general line of treatment followed in such cases is:

1. Intravenous access and fluids to restore fluid status.
2. Pain relief by intravenous route.
3. Patient is kept nil by mouth until the cause has been ascertained.
4. Routine blood tests like a complete blood count, electrolytes and a serum amylase and lipase are done in addition to other tests.
5. An ultrasound of the abdomen which may show a bulky pancreas or any other abnormality like free fluid in the abddomen.
6. If pancreatitis is ruled out, she may be scheduled for an endoscopy which will confirm either gastritis or peptic ulcer.

Early diagnosis and treatment is important here as delay will cause totally preventable complications which otherwise might prove to be serious.

Wishing your friend a speedy recovery.

Dr Geeta
