What Causes Vomiting And Drooping Eyes After Head Injury?

My sister hit her head last night and was knocked out. She was taken to the ER. After waking up she did vomit a couple times. They scanned her and said no bleeding. They let her go once vomiting stopped. Last night she woke up a few time and has been saying strange things. Her right eye is drooping also. She hit the right side. Should she go back for another scan.

Hello. Thanks for asking from HCM.

I can understand your concern. It is good that her first scan was normal. Her drooping of right eyelid and irrelevant talk need evaluation. You must consult ER again. Repeat CT head is needed to rule out delayed bleeding or edema in brain. In case of head injury, first 72 hours is very important time. Delayed bleeding or increase in bleed can occur during this time. Patients who are suspected to be of dangerous head injury must be observed very carefully during this time.

As your sister had multiple episodes of vomiting, drooping of right eyelid, irrelevant talk, it could be due to dangerous head injury. She must be carefully observed for 72 hours. Look for other signs also like

: fits
: bleed through ear/nose/throat
: loss of consciousness for more than 5 minutes
: confusion, disorientation
: persistent headache not responding to painkillers

If repeat scan shows fracture only, it can be managed conservatively by giving painkillers like Diclofeac sodium. If scan shows bleed in brain, it may be evacuated surgically if causing compression over brain. Drooping of eyelid could be due to local injury and swelling or it could be due to IIIrd cranial nerve injury. Local injury will heal in 10 to 14 days and she will improve.
While IIIrd nerve injury may take time upto 3 months or it may never heal. It can only be confirmed by examination of patient by neurologist.

My advise to you is you should not wait at home and consult doctor. At home, you can

: Keep her head at higher level than chest
: Turn head to side in case of vomiting
: Do not give painkiller which cause sedation like opioids
: You can apply icepacks at injury or swelling site

Hope it will help you. Thanks. Take care.
