What Causes Vomiting, Nausea, Bloating And Discomfort In The Stomach?

I have had a long history of stomach issues. I vomit and feel nauseous all the time, it is worse in the mornings when I wake up. I have extreme bloating and discomfort in the top part of my stomach. Sometimes I swell so much that wearing a bra is impossible. I have been to many doctors and there has been no resolution. I have a hiatal hernia that was discovered during one test and that has been all found so far.
