What Causes Watery Semen Discharge?

When ejaculating, no sperm comes out, just clear liquid. I also have low testosterone. I am on trt taking 200 ml of testosterone a week via injection from a dr. I just read up on Varicoceles. I believe this might be the problem. Is it? If so, would surgery help?

1. Is there pain in the scrotum?
2. Is there any swelling in the scrotum?
3. do you see any raised waves in the scrotum?

I would like to give you some tests:

1. Scrotal Ultrasound
2. Semen Analysis
