What Causes Weight Gain For A 78-year-old, Although He Eats Relatively Less?

My brother in law (78 years old) has been on different cancer drugs for 6 years, for kidney cancer. He had an attack or pancreatitis a month ago, and was taken off his drug then. He was admitted to the hospital 4 days ago for extreme pain, which everyone including the hospital said required follow up, which he will receive tomorrow. My question is this: Although he has eaten practically nothing for 4 days, he has gained almost 10 pounds. What could be the reason?

According to the history of his comorbidities thi situation might be from the liquids his organism is collecting as a result of his general conditions
However it is hard to say online about the exact cause.
