White spots or patches which you are talking about on inner surface of mouth are due to nicotine as you have longstanding history of nicotine use in the form of guttka.
Benzedamine, chlorhexidine mouth wash are condidered effective treatment agents for this.
You have to maintain a good oral hygiene with no use of gutka or any other such substance.
Avoid hot and spicy food which can increase the pain.
Antifungal drugs like fluconazole should be used to prevent candidiasis which is a disease similar to your symptoms.
Spots will subside gradually with such combination of factors.
Hope your concern has been resolved.
Best Wishes, Dr. Harry Maheshwari
