What Causes Yellow Discoloration On The Skin And Yellow Eyes?

My Dad is 89 years old, and I have noticed that his skin and eyes are yellow. He has congestive heart failure and is a diabetic. I was wondering what could be causing this and should I take him to the ER? He doesn t say that he is in any pain. Thank you
