What Causes Yellow Vaginal Discharge During Pregnancy?

I am 13 weeks pregnant and I just experienced pale yellow discharge not much of it tho and for the first time then I went toilet wiped and had slight blood, I put a pad on none on the pad and none when I went toilet the second time should I be worried?

I really appreciate your concern, it may not be anything to worry but it can not be taken as granted and better to get done the ultrasonography just for safety of conception hope this information helps.
