These CT scan findings are suggestive of
small vessel disease, which is related to uncontrolled
high blood pressure or diabetes.
From the other hand, migraine can lead to such changes.
For this reason, I would recommend you to closely monitor her blood pressure values and start anti-hypertensive therapy in case of
uncontrolled hypertension. Taking daily aspirin 80-100mg is necessary (if no contraindications), to help stop the progression of these changes.
There is no treatment for such changes, just prevention of further progression.
In case her blood pressure values are normal, I would recommend starting a
calcium channel blocker (verapamil) or topiramate to help improve her headache. But it would be better consulting first with a
neurologist in order to determine the exact type of headache and proper treatment.
