What Could Cause Persistent Neck, Shoulder, And Hand Pain Despite Any Improvement From Exercise?

hello, for almost a year and half i ve had pain in my neck, shoulder, and hands. my cervical mri is normal and no anomaly is shown. most of my pain is located in my left shoulder and irritates to both of my hand especially my left hand. I m a typist and I also play guitar. The Electromyography on both of my hand were normal, except for a minor involvement in the cubital tunnel in my left hand. my blood work is also normal, except for an E.S.R 1 hr of 30. for three months i have been working out and stretching out my shoulders. however, so far no improvement has occurred. i ve been to four neurosurgeons and an Orthopedic. none of them were able to help. i ll be glad if you could have help and guide me to get rid of this condition.
