What Could Rubbery Blood Filled Lump Discharge From Vagina Indicate?

So my friend recently started spotting like she was going to start her period, then the blood came and she used a tampon like she regularly would. But, when she later pulled the tampon out, she felt something fall out of her vagina. And when she wiped it it was a bit rubbery lump with blood and some dark colored substance on it. It was about the size of two quarters. Also, she had a bad fever the day before it happened and had diarrhea. Her mom thinks it was TSS and it was a tampon that skin grew around, but my friend thinks it might have been a miscarriage... I need help
