What Could The Red, Scaly, Itching Blotch On The Hand Be?

I don't know what a red scaly itching blotch on the hand right below the wrist (on top) may be. I hav ehad it about two months. Creams have not helped. Hydrocortisone has not helped. My rheumatologist said I should probably get a biopsy. Couyld it be a rash or leading to something else?
