What Do Cramps Under The Rib Cage On The Left Side Indicate?

I have been having a cramp like pain on my left side under my rib cage all day. Now I’m feeling it further up closer to my chest and I feel like I have a pinched nerve type pain in my neck & left shoulder. Should I be concerned? I have a history of GERD and shortness of breath.
