What Do Fever, Chest Tightness And Coughing Up Of Bloody Substance Indicate?

my daughter coughed up a chunk of tissue that was bloody and hard. she has had a fever on and off and a tight chest. she says her throat hurts. it was a large piece of tissue maybe an inch long. this has happened twice today. I told her to get into the doctor immediately. what are some causes of this because my mind immediately jumps to the worst possible conclusions.
