General & Family Physician 's  Response
Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

Infection is the cause of blood in the urine. There are the other causes in some persons. The exact is not found. Imaging of the urinary tract is required to rule out any structural changes as a cause.

Urinary tract infection would not cause the pain, swelling, and fatigue that you are experiencing. Blood tests such as complete blood count, ESR, thyroid screen, vitamin b12 and folate levels, liver and kidney function tests are needed.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
