Hi,I am Dr. Shanthi.E (General & Family Physician). I will be looking into your question and guiding you through the process. Please write your question below.
My Dr ordered labs one of them was an Epstein Barr test which came back positive does that mean I have mono. And second test was kappa free light chain that number is high 54.10 kappa lambda free light chain ratio number is also high 270.50 and last lambda free light chain is low at 0.20 when she did a protein urine test it was about 200.i know if that number gets to 300 she was going to start chemo for multi myeloma . So my question to you is do these kappa numbers lean towards the myeloma diagnose? Thanks Lori
In my opinion, your test results may be leaning towards a diagnosis of Multiple Myeloma. Increased kappa free light chains and kappa lambda free light chain ratio, may point to the diagnosis of multiple myeloma. Increased urine protein Of 200 mg/day is also on higher side (normal being up to 150 mg/day).
However, still the final diagnosis may be based on finding of increased plasma cells in Bone Marrow. In CBC some of the plasma cells leak in blood causing a low count of other cells, particularly red blood cells. Also, the presence of Bence Jones protein in urine are seen in 70-80% of cases of multiple myeloma.
Take care. Hope I have answered your question. Let me know if I can assist you further.
