White spots that appear on the foreskin/shaft aren't always a cause of concern since common causes like penile papules can disappear on their own within a few days or weeks, however, if symptoms last longer and you are concerned then please visit your doctor to exclude causes like:

penile thrush [penile yeast infection]

balanitis [fungal or bacterial infection]
- any
sexually transmitted disease [syphilis, chlamydia, gonorrhea, warts]: since pain during sex is there

molluscum contagiosum [viral skin infection]: occur via direct skin to skin contact or having sex with the infected partner
2. most of the times white spots on the foreskin could be sign of STDs or an infection and if left untreated can lead to long-term complications [easier infection with HIV], so my advice is
- sexual health checkup is essential and finish the course of treatment advised.
- advice your sexual partner also to go for screening/treatment.
- use a condom every time you have sex.
