What Does this blood Test Result Mean?

Hi Doctor, My mother had blood test and her Hemoglobin level is 8.0 gm/dL since last few weeks. and other unusual levels are TOTAL WBC COUNT-3720 cells/cumm, RBC COUNT-2.20 Millions/cmm, PCV (HAEMATOCRIT)-23.3 %, MCV- 105.9 fL, MCH-36.4 pg and ESR in boarder line-20 mm/1st Hour. Could you please advise further.. Thanks,

Hi thanks for asking question.
I can understand your concern.
First of all your MCV is high, it is normally should not exceed 100.
Second your hemoglobin and hematocrit level is also low.
So here strong possibilty that it could be case of megaloblastic anemia.
You must done your blood peripheral smear examination.
If megaloblastic changes present then hypersegmented neutrophils with macrocyte can be seen.
The confirmation of megaloblastic anemia then done with vitamin B12 and folic acid estimation.

Peripheral smear examination might reveal few normoblast.Then reticulocyte count done to rule out hemolytic anemia.

So first of all these two cause has to be ruled out.
Then after still cause not clear then megaloblastic picture might be because of liver disease, hypothyroidism or aplastic anemia like cause that has to be ruled out.
Still no cause clear then bone marrow examination can be done to rule out MDS.
