General & Family Physician 's  Response
Hi,

Usually, the scabs under the lip that hurts is a cold sore, unless he has been eating or drinking any food/drink that caused irritation of the lip (frizzy, acidic or spicy).

Do not get worried unnecessarily as it is not that serious. It will be healing on its own. He should not keep peeling the scab to help healing.

Apply cold sore cream locally until improvement.

Wish fast recovery.
