Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.
As per your complain it seems that a large sore on the side of butt cheek can be an abscess that has not yet started draining. It occurs most commonly due to hair follicle infection while other causes can be infected gland in the area or infection in the skin leading to pus formation under the skin. I would suggest you to consult a Proctologist or Anorectal surgeon or you can even see a general surgeon and get it evaluated. He can advise you a course of antibiotics, anti-inflammatory painkiller, and topical application of antibiotic, steroid, and numbing ointment. Do warm compresses as it can ease in draining it off. If it does not improve or start worsening then incision and drainage can help in faster resolution.
Hope I have answered your query. Let me know if I can assist you further. Regards, Dr. Honey Arora
