General & Family Physician 's  Response
Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advise.

It's cancer growth and fluid exudate in peritoneal cavity which is causing abdominal distension. If it is progressing despite starting chemotherapy then consult treating doctor immediately. If it has infiltrated surrounding organs then surgical removal followed by peritoneal chemotherapy is the treatment option.

Hope I have answered your query. Let me know if I can assist you further.
Regards,
Dr. Hiren Hirpara
